As far as I am aware facelift diesels were fitted with the 6sp box and the more powerful engine straight away whereas the 3.0i petrol model didn't get the new box until much later, not sure when they got an engine update if at all on the e53.

Temp gauge reading that low would definately point to the thermostat being stuck open although no hot air could point at either an air lock or the water pump not working as it is the pump that pushes the hot water through the system including the heater matrix.
